Health Care Benefit Plan

The Health Care Benefit Plan includes dental service and prescription medication coverage to eligible University of Toronto Casual Unit employees. Eligible employees may claim up to $400 for dental services and up to $400 for prescription medications per calendar year.

 

Eligibility

  1. You are a University of Toronto Casual Unit employee with an appointment of 20% or more (or the equivalent of 7.25 hours per week) in any four consecutive months in the past 12 month period; or
  2. You are a University of Toronto Casual Unit employee who works on an intermittent or irregular basis with at least five scheduled shifts per month in any four consecutive months in the past 12 month period; and
  3. You are not eligible for health care benefits through another University of Toronto employment group, retiree benefit plan, or student benefit plan.

  • WSIB Form 8, “Health Professional’s Report.” You will be provided a copy for yourself and a copy to provide to Health and Well-Being.
  • WSIB Form 6 “Worker’s report of Injury/disease” online and submit. Also, provide a copy to Health and Well-Being.
